Pheochromocytoma is seldom the cause of secondary hypertension. The presentation without eleva-tion of catecholamines and their metabolites, is still a less frequent condition. In case of high clinical suspicion, measurements should be repeated, specially intracrisis because of the paroxysmal release of these substances. We report the clinical case of an adult female that remained 10 years without etiological diagnosis, despite laboratory tests and diagnostic imaging procedures were performed. Finally, an abdominal ultra sonogram revealed a suprarenal enlargement on the right side. This finding was confirmed by abdominal CT scan and MIBG scintillography. The right adrenal mass was excised without incidents. The diagnosis of pheochromocytoma was confirmed by hystological examination.
