On the last years, basic and clinical research, have demonstrated to all of us, that when cardiopulmonary bypass is on, a real "storm" of chemical and sub cellular order appears. Cascade reactions, with dozens of mediator release, local and systemic hormones, cytolytic substances from leukocyte source and powerful messengers, which sometimes produce a serious panendothelial injury and multiple organ dysfunction. Systemic inflammatory response syndrome, some-times shows hyperdynamic pattern currently named vasoplejic syndrome. Knowledge of the trigger of these inflammatory response, give us facilities for development of appropriate therapeutic and preventive strategies or decrease its deleterious effects.
