Efficacy of trimetazidine in the chronic stable angina assessed by excercise stress test

The aim of this study was to evaluate the antiischaemic action of trimetazidine (TMZ) in the chronic ischaemic cardiopathy. We studied 30 male subjects (mean age years, 52.2 ± 1.1) with stable chronic angina (mean duration 38.8 ± 3.7 months). The trial was divided into the following periods: placebo (14 days), TMZ (21 days), placebo (7 days) with a 60 mg/day dosage, t.i.d. An exercise stress test was done at the end of each period. The total work increased 31 % under the action of TMZ (4416 ± 18 vs 5578 ± 221 kg/min; p< 0.001) and the duration of the effort 14 % (650 ± 18 vs 740 ± 19 s; P < 0.001). The ischaemic threshold was delayed 15 % (620 ± 18 vs 740 ± 19 s; P < 0.001). The double prQ- duct increased from 26.800 ± 626 to 28.844 ± 526 (p< 0.001). In the final placebo period the results presented a significant reduction withouth reaching the initial values. Blood pressure and cardiac frequency had no significative modifications in each period. The reduction of the effort capacity in both placebo periods allows us to attribute the obtained benefits to the TMZ action, and excludes the possibility of a spontaneous evolution of the pathology. We conclude that TMZ has demonstrated an antiischaemic efficacy in the chronic stable angina confirmed by a greater effort capacity.
