Hyperinsulinemia Predictors in Patients with Essential Hypertension

Hyperinsulinemia (HIN)has been linked to the arterial hypertension (HTA) associated to diverse metabolic alterations and it is consider to be a risk factor of atherosclerotic illness.
Objective
The aim of the study was to analyze metabolic profile to detect HIN predictors in hypertensive patients.Material and methodsSeventy five patient HTA were studied: 49 men and26 postmenopausal women, age 54±10, without damage organ target without medication, body mass index (BM) until 30 kg/m2, and a group of 33normotensive (NT), 16 men and 17 women, age 52±10. Plasmatic cholesterol, HDL-c and triglyceride (TGC) were determined. Oral glucose tolerance test, was performed with an overload of 1 g/kg. Glycemia and insulinemia were measured at basal 0', 30',60' and 120'. These results were expressed as area under the curve (AUC) (LUI/ml x 2 hours and mg/dl x 2 hours, respectively).
Results
Data were analyzed by means of t of Student test, Mann-Whitney and multiple regression. The AUC of insulin in HTA: 7,716±4,847 versus NT: 4,288±2,352 (p < 0.001) and the AUC of glycemia 14,646±4,193 versus 12,854 ± 1,601(p<0.05). Thirty two percent of HTA showed AUC of in values that overcame the percentile 95 of the control group (8,973μUI/ml x 2 hours), this group HTA was consideredHIN.The technique of multiple regression forsteps generates two models: a) predictive factors (IN 120) were BMI and TGC r: 0.48, r2:0.228 (p<0.00009); b) predictive factors (AUC in) were BMI and age r: 0.50, r2:0.25 (p < 0.000032).
Conclusions
The variable age, BMI and TGC, may predict the hyperinsulinemia in HTA with acceptable statistical significance.
