Coronary Artery Spam after Cardiac Transplant

It is well known that coronary artery disease is more frequent in cardiac transplant population. Few cases of coronary artery spasm have been reported but almost all of them in relation with previous coronary lesions and in a chronic phase. The case of a woman with three episodes of hemodynamic unstability and transitory ST segment elevation 18 hours after heart transplant is presented. Coronary arteriography was performed and reported as normal. The clinical situation was controlled with intravenous nitroglycerin and nifedipine. In the long term, the patient was treated exclusively with diltiazem and no further episodes occurred.
