There are very few reported cases of patients with coronary spasm responsible of lethal ventricular arrhythmias. Even more scarce is the information about those patients that needed the implantation of a cardiodefibrillator (CD). In an extensive search, among the most important centers of electrophisiology of the word, updated at the time of this publication, only 13 cases were reported, two of them in our country (Hospital Castex de Buenos Aires, and in Rosario, Santa Fe).The objective of this report is to present a case of a female patient with a documented refractory coronary spasm complicated with ventricular tachycardia and ventricular fibrillation, that needed an implantable cardiodefibrillator with heavy pharmacological support. This would constitute a case of malignant coronary spasm.
